Effective Date: September 25, 2026
This notice describes how medical information about you may be used and disclosed, how Bliss Healthcare Solutions protects your health information, and how you can access and exercise your rights regarding that information. Please review it carefully.
Your Health Information and Your Rights
Bliss Healthcare Solutions is committed to protecting the privacy and security of your protected health information. Protected health information, or PHI, generally includes information that identifies you and relates to your health, healthcare, or payment for healthcare.
You have certain rights regarding the health information we maintain about you. Some of these rights may be subject to limitations permitted by law.
Access Your Health Information
You may request access to or a copy of your medical record and other health information we maintain about you. You may request an electronic or paper copy when available.
We will respond within the time required by applicable law. When permitted, we may charge a reasonable, cost-based fee for providing copies.
Request a Correction
If you believe information in your medical record is incorrect or incomplete, you may ask us to amend it. We may deny your request in certain circumstances permitted by law. If we deny the request, we will provide an explanation in writing.
Request Confidential Communications
You may ask us to communicate with you in a particular way or at a particular location. For example, you may ask that we contact you only at a specific phone number or send correspondence to a particular address.
We will accommodate reasonable requests as required by law.
Request Restrictions
You may ask us not to use or disclose certain health information for treatment, payment, or healthcare operations. We are generally not required to agree to every requested restriction.
If you pay in full out of pocket for a healthcare service or item, you may ask us not to disclose information about that service or item to a health plan for payment or healthcare operations purposes. We will honor such a request when required by law unless another law requires the disclosure.
Request an Accounting of Disclosures
You may request an accounting of certain disclosures of your health information made during the period permitted by law.
The accounting generally does not include disclosures made for treatment, payment, healthcare operations, disclosures you specifically authorized, or certain other disclosures excluded by law.
Receive a Copy of This Notice
You may request a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.
Choose Someone to Act on Your Behalf
If you have given another person legal authority to act for you, such as through a healthcare power of attorney or legal guardianship, that person may exercise your privacy rights as permitted by law.
We may verify that person’s authority before responding to a request.
File a Privacy Complaint
If you believe your privacy rights have been violated, you may submit a complaint to Bliss Healthcare Solutions using the contact information provided at the end of this Notice.
You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
Bliss Healthcare Solutions will not retaliate against you for filing a privacy complaint.
Your Choices About Your Health Information
In certain situations, you may tell us how you would like your health information to be shared.
Family Members, Friends, and Caregivers
You may tell us whether we may share relevant health information with a family member, friend, caregiver, or another person involved in your care or payment for your care.
If you are unable to communicate your preference, we may use professional judgment and applicable law to determine whether a limited disclosure is in your best interest.
Disaster Relief and Emergency Situations
We may share limited information with organizations assisting with disaster relief or emergency response when permitted by law and appropriate under the circumstances.
Marketing and Sale of Health Information
We will obtain your written authorization before using or disclosing your protected health information for marketing activities that require authorization under HIPAA.
We will not sell your protected health information without your written authorization when authorization is required by law.
Psychotherapy Notes
If Bliss Healthcare Solutions maintains psychotherapy notes that receive special protection under HIPAA, most uses and disclosures of those notes will require your written authorization unless an exception under applicable law applies.
Written Authorization
For uses and disclosures not otherwise permitted by law or described in this Notice, we will request your written authorization.
If you provide authorization, you may revoke it in writing at any time. Revocation will not affect actions already taken in reliance on your authorization.
